When requesting information you _must_ include the following:

-- full name

-- username or member # if from a group such as PAFOA, WVCDL, VCDL, SCCC, active duty military / reserves / guard, LEO's or members of the Patriot Guard

-- please confirm you are age 20 or older (Yes / No)

-- your zip code and / or the zip code of the potential class location

-- telephone / cell phone (please specify if you provide a business number so that I may be discrete)
